- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Qualifications:
- Active Physical Therapist (PT) license in the state, in good standing
- Minimum of 1 year of recent Home Health PT experience (field experience strongly preferred)
- Proficiency with Home Care Home Base (HCHB) preferred
- Thorough working knowledge of OASIS documentation and home health assessment tools
- Strong clinical reasoning and functional assessment skills
- Reliable vehicle, valid drivers license, and current auto insurance
- Ability to manage a flexible, field-based caseload independently
- Effective communication, time management, and organizational abilities
Key Responsibilities:
- Conduct comprehensive patient evaluations, including OASIS assessments as required
- Develop and implement individualized therapy plans targeting functional goals
- Carry out skilled physical therapy interventions: exercise programs, gait and transfer training, neuromuscular re-education, and fall prevention
- Modify interventions based on patient progress and collaborate closely with other healthcare professionals
- Provide in-depth education to patients and caregivers regarding safety, device use, and home exercise
- Document all care in the Home Care Home Base (HCHB) system accurately and on time
- Participate in case conferences, interdisciplinary meetings, and ongoing professional development
